Hospice Care

TRU Hospice, a program of TRU Community Care, encompasses specialized, compassionate end-of-life care designed to meet the physical, emotional and spiritual needs and preferences of people at any age, with any type of end-stage disease.

Palliative Care

TRU Palliative Care provides support for you and your primary physician to optimize your quality of life. It is a collaborative approach to managing your physical, emotional, social, and spiritual needs and a shared effort to maximize your health and minimize your symptoms.

TRU PACE

TRU PACE is an innovative model that enables individuals who are 55 years old or older and certified by the state to need nursing-home-level care to live as independently as possible. TRU PACE (Program of All-inclusive Care for the Elderly) coordinates and provides all needed preventive, primary, acute, and long-term care services so that older individuals can continue living safely in their community.

Grief Services

TRU Grief Services provides extensive resources for people of all ages and phases in the grief process. Whether you are looking for personal guidance or walking alongside someone who is, contact us to see how we can help.

Dementia Support

At TRU Community Care, we understand the challenges that come with supporting a loved one living with dementia. Our dementia support program is part of a national Medicare demonstration project called GUIDE—Guiding an Improved Dementia Experience. Through GUIDE, we provide in-home, coordinated care to help people live safely and meaningfully in the place they call home. We’re here to walk beside you—offering expert guidance, emotional support, and practical resources.

Memory Care

In 2020, TRU acquired Landmark Memory Care, a provider of care for those living with Alzheimer’s and dementia. Several TRU PACE participants already reside at Landmark, which provides care to 18 residents each day. This move represents an opportunity for TRU to expand Alzheimer’s and dementia care services in the community. 

The Conversation Project

The Conversation Project provides workshops, seminars, coaching, and educational materials for individuals and groups on advance care planning and having a conversation about future medical care, and assistance with filling out a Medical Durable Power of Attorney.
