Who Qualifies for TRU PACE

Many families aren’t sure at first whether PACE is the right fit — and that’s okay. To enroll in TRU PACE, individuals must meet the following requirements:

How TRU PACE Works

Enrollment begins with a conversation — not paperwork. We start by learning about your goals, daily routines, and what matters most to you.

Next, the care team completes an assessment to understand medical, social, and home support needs. Once enrolled, care is provided through TRU PACE-contracted providers, with all services coordinated by one team.

Appointments, therapies, and transportation are scheduled for you, and emergency care is covered at any hospital.

TRU PACE

TRU PACE (Program of All-Inclusive Care for the Elderly) helps older adults stay safe, supported, and active in their community, without having to manage care alone.

TRU PACE brings together preventive, primary, and long-term care in one coordinated plan. Participants receive medical care, transportation, and day center support so they can live as independently as possible — with one team guiding care every step of the way. Interpreter services are available at no cost.

Managing health, appointments, and daily needs can feel overwhelming for both older adults and their families. TRU PACE supports people with complex health needs who want to remain in their homes, while helping caregivers navigate decisions with greater confidence and less stress.

TRU PACE serves families across Boulder and Broomfield counties and portions of Weld, Adams, and Jefferson counties. 

You must live within the TRU PACE service area to enroll. Eligibility is determined by ZIP code. Our team is happy to help you check and answer any questions along the way.

A Note About Coverage

Understanding coverage helps families make informed decisions with confidence. TRU PACE participants receive all non-emergency care from TRU PACE network providers. Using providers outside the network without prior approval may result in personal costs. 

Participants may disenroll at the end of any month. Our team is always available to talk through coverage questions before or after enrollment.

No. PACE is designed to help you stay at home. Care is coordinated through the day center and home visits.

PACE becomes your managed care provider for Medicare and Medicaid services if you qualify.

Your schedule is based on your needs. Some participants come several times a week for care or activities, while others come less often.
